Transition is the time when services shift for individuals with disabilities—from supports provided by the school system to those available in adulthood. It’s not a single moment, but a gradual process.
At Together We Navigate, we know this transition can feel overwhelming—but you don’t have to navigate it alone. Designed for parents and caregivers of teens and young adults with disabilities (ages 14 and up), this coaching program offers a complete and compassionate approach to preparing for adulthood.

Our team not only helps you understand available services—we also assist with completing applications, gathering necessary documents, and providing hands-on support through administrative and advocacy work.
Whether your goal is independent living, employment, or simply knowing what options exist, we’re here to guide the way.

Cost: $60/hour
Payment options: Family Support Funds, NOW/COMP Waiver, or Private Pay
As your child moves toward adulthood, you may find yourself facing new systems, unfamiliar choices, and critical timelines. Our team is here to walk with you every step of the way, helping you prepare for what's ahead and make informed decisions that lead to long-term success.
Through 1:1 weekly parent coaching sessions, you will learn how to navigate topics such as:
High school supports, IEPs,
and transition planning
Exploring vocational programs and job supports
Funding pathways, including Medicaid
waivers and family support funds
Estate and guardianship planning for future care
Understanding and applying for Social Security (SSI/SSDI)
Transportation options and
independent mobility planning
Building a trusted support team for adulthood
